Using the Link+ monitoring gateway, patients are able to take their blood pressure, glucose, temperature, and more with the easy to use at-home tools. The data from those tools is uploaded automatically and can be accessed securely by your doctor. Your doctor will also be alerted to any changes in those readings that may cause concern.
The Plessen Tele-Concierge device and tools are covered by Medicare and other health insurance.

Using the Link+ powered proprietary, HIPAA-compliant, FDA-cleared smart home gateway, the device is designed to be placed at the patient’s home. The simple, user-friendly interface eliminates the need to download any apps or require long and complicated Bluetooth pairing processes. Patients can use all of their home monitoring devices (blood pressure, pulse oximeter, glucometer, scale, etc.) and have their results uploaded directly to the portal without any further action on their part.
