Using the Link+ monitoring gateway, patients are able to take their blood pressure, glucose, temperature, and more with the easy to use at-home tools. The data from those tools is uploaded automatically and can be accessed securely by your doctor. Your doctor will also be alerted to any changes in those readings that may cause concern.

The Plessen Tele-Concierge device and tools are covered by Medicare and other health insurance.

Patients can use all of their home monitoring devices (blood pressure, pulse oximeter, glucometer, scale, etc.) and have their results uploaded directly to the portal without any further action on their part.
